MARY POTTER COMMUNITY HOSPICE AT PORIRUA

The Community Hospice was established in Porirua in early 2003. Previously the needs of the Porirua residents were met through locally-based care co-ordinators and a palliative care physician. Additional services were accessed through the Wellington-based Mary Potter Hospice staff. Mary Potter Hospice is very pleased to now be able to offer an on-site multi-disciplinary service for the people of Porirua.

The Community Hospice is located on the grounds of Keneperu Hospital in the Community Health Service building, Ambulance Drive.

Palliative Care Co-ordination
There are two Palliative Care Co-ordinators based at the Porirua Community Hospice to serve the local community. The Palliative Care Co-ordinators visit people and their families in their homes in the community and work with other local health professionals to ensure that the best and most suitable palliative care is given.

Palliative Care Physician
A specialist palliative care doctor is available to visit patients in their homes and to liaise with the Mary Potter Hospice multi-disciplinary team, the GP, and Community Health Services.

Occupational Therapy
An occupational therapist is available to visit people at home to help them to remain as involved as possible in the activities they enjoy. Support can be given for

  • Management of work, self care or leisure activities
  • Management of daily activities
  • Equipment needs
  • Relaxation & stress management

Social Work Services
A social worker is based at the Porirua Community Hospice and is available to assist people and their families. The social worker is available to visit people at home and support can be given for

  • Advocacy and information
  • Patient and family support
  • Grief and bereavement issues

Counselling can also be given at the Community Hospice or at home.

All services offered by Mary Potter Community Hospice are free. Fundraising supports our work in the Porirua community.
